Financial Information

An initial payment is due on the day treatment is provided.  The amount will be discussed at the time of the consultation/evaluation.  Check, cash, Visa, MasterCard or Discover are acceptable methods of payment. 

It is important for you to know that even if you are covered by dental, medical or accident insurance, our professional services are rendered and charged to you, not your insurance carrier.  We can submit a claim to your insurance carrier for your treatment and request that you provide us with correct and/or updated insurance information. 

Since we are not providers for all insurance companies, and some programs provide limited coverage, we urge you to be fully aware of the provisions of your plan;  we assume no liability or responsibility for knowledge of your plans provisions or limits.  If requested, we will submit for predetermination or pre-authorization of benefits prior to treatment and respect the disclaimer made by insurance carriers that “predetermination or prior authorization of benefits is no guarantee of payment or benefit allowance.”   Institute of Facial Surgery St. Paul is not responsible for misinformed benefits given by your insurance carrier and ask that you contact them if you disagree with the benefit allowance paid.
